How to signup:

  1. Click here to go to the offer page
  2. Then click on “Get This Freebie”
  3. Answer the short questions
    Such as policy agreement, birth year, etc
  4. Enter your CVS ExtraCare Card number
  5. Click “Continue”
  6. Also answer the following questions
    These will help determine if the panel is a good fit for you
  7. Once completed, submit your answers
